Introduction

Mrs Margaret Adams has smoked 20 cigarettes per day for 45 years (equating to 45 pack years). Over the past 5 years she has become increasingly breathless but has only seen her GP twice in that time with bouts of ‘bronchitis’. On these occasions she has refused any request to stop smoking. You have been asked to see her.
